- Individualised dietary advice based on patient’s requirements

Dietetic intervention will usually entail 3-4 appointments
(subsequent appointments to be booked in clinic depending on individual progress)

Pre-assessment Questionaire & Food Diary
Please fill in the attached questionnaire and the food diary most applicable to your appointment type and return them 5-7 days in advance of your appointment. This is to ensure we make the best use of our consultation time which is 45 minutes as I can pre-assess your condition and requirements. If you are unable to complete them in advance, please be advised that an additional appointment may be required to complete the initial assessment.
Payments
Transaction receipts are provided at each appointment and a full formal receipt suitable for reclaiming from health insurance and also revenue.ie is provided on discharge.
Payment is taken when you book your appointment via a secure online payment system.
A non-refundable admin charge of €7.50 is included in your appointment fee.
Cancellation Policy
I operate a strict 48 hour cancellation period. The full fee for your assessment or review appointment will be applied for late cancellation.
